It is our pleasure to inform our patients and the San Antonio community that we have extended our practice from inpatient hospital care by adding a clinic-based practice under CareFront Medical Group.

Meet Our Providers
Dr. Lajja Patel, M.D.
Dr. Patel is a Board Certified Internal Medicine Physician. She completed her medical education from Ross University School of Medicine in 2009 and completed her Residency training at University of Kentucky in 2012. She has been a practicing Internal Medicine physician for the past six years. Dr. Patel grew up in Chicago, Illinois, and now lives in San Antonio. She has served as the Medical Director for several Hospitalist Programs in the past and currently serves as the Medical Director for MedCede Physician Services in San Antonio, Texas.
Constance Henry, AGPCNP-C
Constance has been in the medical community for 10+ years now. She has been a LVN, RN, and is now a Nurse Practitioner while simultaneously working on her Doctorates in Behavioral Health. She is a Texas Girl through and through, hailing from Temple, TX and currently resides in our great city of San Antonio. She has a wonderful child who she spends her time off with and does family oriented activities. Constance is a firm believer in her Faith and the well-being of her patients. She is a wonderful asset to CareFront and is always willing to help out her fellow humans.
